60 percent of Chico State employees have not received their W-2 forms as of Wednesday.

The W-2 forms were prepared and mailed by the State Controller’s Office by Jan. 24, according to an employee campus-wide email. Chico State has informed the California State University’s chancellor’s office and State Controller’s Office about the W-2 forms.

There is investigation at the controller’s office about the unreceived W-2 forms, said Sarah Langford, a spokeswoman for Chico State. The university has made a request to resend W-2 forms.

“It’s urgent because it’s tax season,” she said. “For people that need a tax return. They may want to process their taxes more quickly.”

Employees can request a duplicate or unofficial W-2 form through Chico State’s human resources office. Unofficial forms will be available within 24 hours of the request. Duplicates can take up to six to eight weeks to arrive with an $8.50 fee.

Employees who do not receive their W-2 forms by Feb. 14 can contact the Internal Revenue Service office at 800-829-1040. According to its website, employees must have the following information available:

• Employer’s name, address, city and state, including zip code and phone number.
• Dates of employment.
• An estimate of the wages you earned, the federal income tax withheld and when you worked for that employer. The estimate should be based on year-to-date information from your final pay stub or leave-and-earnings statement, if possible.

Chico State hasn’t determined which kind of employee — faculty, staff or student — has been affected the most, Langford said.

“We’re doing everything that we can to provide the service possible during this time,” she said. “It’s a frustrating process.”
